We will email you patient forms to review and fill out prior to your appointment. These forms will also be available at the time of your appointment. You’ll also need Florida proof of residency/ID. Florida residents are granted same day approvals 99% of the time. Part-time residents, or those without a FL ID, will have a 2-3 week wait period before approval. Medical Records are able to be obtained through our release forms, by doctor’s request.
What conditions qualify?
At your initial evaluation the doctor will review your health conditions, discuss your treatment options, and document all requirements under Florida law.
MMJ Care MD’s physician will enter the qualifying patient’s certification and orders into the Medical Marijuana Registry following the initial appointment. The Office of Medical Marijuana Use will then email the patient a login.
In order to receive the required ID card, a patient and/or caregiver must submit a completed application to the Office of Medical Marijuana with a copy of State ID/proof of residency, and the required fee ($77.75). Our staff will help you to complete the entire application process during your first visit. Once you leave our office, all you have to do is wait for an email from the Office of Medical Marijuana Registry. FL Residents will receive the approval email & be able to shop immediately in 99% of cases (the state may also request any given patient to do go through a 2-week manual review at random). Part-time residents will receive an approval email & be able to shop within 5-10 business days.
You do not have to wait to receive your physical card to purchase the medical marijuana products. The email from the Office of Medical Marijuana Registry, or your Florida Driver’s License/ID, is sufficient for you to visit a dispensary and obtain your medication. You should receive your actual card in the mail within approximately 30 days.
MMJ Care MD will maintain the required documentation to keep our patients in compliance with Florida State law.
Only state licensed Medical Marijuana Treatment Centers (MMTC), also known as dispensaries, can legally provide medication. For most comprehensive list of dispensaries, please visit our RESOURCE section which provides a link to each Florida dispensary website.
Our doctor will work with you to decide on the best dosing and delivery methods for your needs. An order can be submitted for up to 70 days of medication at a time, but you do not have to purchase your entire order at once. We recommend experimenting with different products and strains to find the best fit before making a large purchase. You will know within the first week of use if a product is effective for your needs.
Your doctor is allowed to make recommendation for 210 day periods at a time. The Department of Health requires a follow-up to ensure that you are receiving proper and effective medical cannabis treatment.
Please keep in mind, that your medical marijuana card must be separately renewed annually with the Office of Medical Marijuana Use. We can help you with the process, but it is the patient’s responsibility to remember & track the renewal date.
