Review and fill out all patient forms (we will email them to you) prior to your appointment. You’ll also need to bring medical records that document your qualifying condition(s) and Florida proof of residency/ID. If you do not have your medical records we have release forms that will request them on your behalf.
What conditions qualify?
At your initial evaluation the doctor will review your health conditions, discuss your treatment options, and document all requirements under Florida law.
MMJ Care MD’s physician will enter the qualifying patient’s certification and order into the Medical Marijuana Registry following the initial face to face appointment. The Office of Medical Marijuana Use (formerly the Office of Compassionate Use) will then email the patient a login.
In order to receive the required ID card, a patient and/or caregiver must submit a completed application to the Office of Medical Marijuana with a copy of State ID/proof of residency, and the required fee (currently $75). This can be done online or via paper application. We will help you to complete the entire application process during your first visit. Once you leave our office, all you have to do is wait for an email from the Office of Medical Marijuana Registry. The email should arrive in about 10 business days and it will contain your medical marijuana card number.
You do not have to wait to receive your physical card to purchase the medical marijuana products. The email from the Office of Medical Marijuana Registry is sufficient for you to visit a dispensary and obtain your medication. You should receive your actual card in the mail in approximately 30 days.
MMJ Care MD will maintain the required documentation to keep our patients in compliance with Florida State law.
Only state licensed Medical Marijuana Treatment Centers (MMTC), also known as dispensaries, can legally provide medication. For most comprehensive list of dispensaries, please visit our RESOURCE section which provides a link to each Florida dispensary website.
Our doctor will work with you to decide on the best dosing and delivery methods for your needs. An order can be submitted for up to 70 days of medication at a time, but you do not have to purchase your entire order at once. We recommend experimenting with different products and strains to find the best fit before making a large purchase. You will know within the first week of use if a product is effective for your needs.
Our doctor is allowed to make recommendation for three (3) 70 day supply of medical marijuana. The Department of Health requires face to face vist with the physician every 210 days. This ensures that you are receiving proper and effective medical cannabis treatment.
Please keep in mind, that your medical marijuana card must be renewed annually with the Office of Medical Marijuana Use. We can certainly help you with the process, but it is the patient’s responsibility to remember the renewal date.
